Updated: Workplace Incident Investigation Report (SU-17) Form
As part of the University’s efforts towards continued reduction in workplace safety risks, substantial changes to the Incident Investigation Report Form (SU-17 Form) have recently been made. The revised form will better help supervisors follow up on workplace concerns via:
- Better capture of key incident details
- Easier identification of likely contributing factors and root cause of an incident
- Guidance for planning follow-up safety efforts
Regarding Non-Employees:
Another major change is that incidents involving students w/ non-employee status, visitors, and other third-parties will no longer be reported using the same SU-17 form. Non-employee incident reporting should be done using the Non-Employee Incident Report Form (SU-17B).
