INSTRUCTIONS FOR VOLUNTARY USERS OF N95 RESPIRATORS

The following instructions apply only to Stanford University employees who are NOT required to wear a N95 respirator, but would like to wear one for comfort-purposes.

  1. Voluntary users complete the Voluntary Respirator Use Agreement form found here.

  2. Voluntary users submit a copy of the completed form to their supervisor for record retention (retain records for at least 1 year). Further training/ fit-testing is not required for voluntary users. For additional consultation, contact EH&S at 723-0448.
